LAS VEGAS — A fire captain who became paralyzed after an apparatus crash 15 years ago died Tuesday from his injuries.

Las Vegas Review-Journal reported that Las Vegas Fire-Rescue Captain and 13-year veteran Thelonious “Theo” Adams, 54, became paralyzed from the neck down after the fire engine he was in rolled over while responding to a false alarm call on Halloween night in 2003.

The department announced Wednesday that Adams died from his injuries.

The driver of the engine was cited for “duty of driver to decrease speed under certain circumstances” after the crash, and it was later found that the engine’s speedometer was in need of replacement, according to department maintenance records.

According to a department Twitter post, all station flags will be flown at half-staff in honor of Adams.
