YORK, Pa. — The National Institute for Occupational Safety and Health Administration has released a report on the deaths of York City Fire Department Firefighters Ivan Flanscha, 50, and Zachary Anthony, 29.
The 57-page report details five contributing factors that led to Flanscha and Anthony’s death in March 2018, including deep-seeded fires that large hoses could not extinguish, inactive sprinklers, previous structure collapses at the building, firefighters entering the collapse zone and a lack of risk vs. gain analysis.
On March 22, 2018, firefighters responded to a fire at the Weaver Organ and Piano Company. The building collapsed when Flanscha and Anthony were on the fourth floor along with Assistant Chief Greg Altland, 52, and Firefighter Erik Swanson, 50, according to the York Daily Record.
Both Atland and Swanson were severely injured during the collapse, and have filed lawsuits against the building engineer for negligence. Both widows have filed wrongful death lawsuits.
